If you wish to advise us / update us of your contact details, please email us your details to info@dchc.ie or ask a family member to help you with this. You should include your PPS number, GMS card number, Date of birth and a contact mobile number
If we have not called you up to 3 days before your scheduled day (see above), you should call the surgery to identify yourself as eligible.
